COVID-19 Self-Assessment
Please review the questions below and perform the self-assessment steps provided no earlier than 1-3 days prior to attending Macstock in person.
Please read each question carefully and answer to the best of your ability as the health and safety of those in attendance as well as their families depends upon it.
If, after taking the self-assessment, you are unable to attend Macstock due to infection, a positive test, or you’ve been exposed to someone with COVID-19, please Contact Me for options.

This “self-screening” is NOT intended to be a medical diagnosis or to take the place of proper testing and/or advice of a medical professional! This is merely a tool to help you make a decision whether you should attend Macstock in person. Please contact your medical provider for any questions you may have about COVID-19 symptoms, COVID-19 testing, or other medical issues that are concerning to you.
Step One: Have you experienced any of the following symptoms in the past 10 days?
- fever or chills
- cough
- shortness of breath or difficulty breathing
- fatigue
- muscle or body aches
- headache
- new loss of taste or smell
- sore throat
- congestion or runny nose
- nausea or vomiting
- diarrhea
- Other symptoms commonly associated with COVID-19
This list does not include all possible symptoms. Per the CDC: Older adults and people who have severe underlying medical conditions like heart or lung disease or diabetes seem to be at higher risk for developing more serious complications from COVID-19 illness.

Continue to Step 2
Step Two: Have you tested positive for COVID-19 within the past 14 days, are isolating or quarantining because of COVID-19 infection, OR are you worried that you may be sick with COVID-19?

Step Three: In the last two weeks, have you been in close contact with someone who has COVID-19?
Per the CDC: “You have been in close contact if you have been less than 6 feet away from an infected person (laboratory-confirmed or a clinical diagnosis) for a cumulative total of 15 minutes or more over a 24-hour period.”
IF YES:

IF NO:
You’re Cleared to Attend
Step Four: Were you tested for COVID-19 3-5 days after your exposure with the close contact and received a negative result?

You’re Cleared to Attend
IF NO:
