COVID-19 Self-Assessment

Please review the questions below and perform the self-assessment steps provided no earlier than 1-3 days prior to attending Macstock VI in person.

Please read each question carefully and answer to the best of your ability as the health and safety of those in attendance as well as their families depends upon it.

If, after taking the self-assessment, you are unable to attend Macstock due to infection, a positive test, or you’ve been exposed to someone with COVID-19, please Contact Me for options.

Thank You
Mike
